New Patients

Dr. Praderio is currently accepting new patients. However, our process for new patient intake has changed.

AS OF JUNE 9, 2022 we will only schedule new patients through our new patient portal. Due to recent shortage of Gynecologists in the area and the high volume of new patient requests, we CANNOT GUARANTEE ANY APPOINTMENT. Because of this, wait times for appointments vary anwhere from 2 weeks to 6 months.  We appreciate your patience and undertstanding. 

Please note that simply submitting a request does not GUARANTEE ANY APPOINTMENT. 

We are no longer able to schedule any New Patients over phone. 

We require a note from your Primary Care Physician (PCP) regardless of the type of insurance you plan to use.  

Here is what to expect from our new patient intake process: 

1. In order to expedite the process, please have your primary physician send us the requested paperwork.  Our fax number is 844.717.5672.  WE DO REQUIRE YOU TO VISIT YOUR PCP BEFORE WE CAN PROCESS YOUR NEW PATIENT REQUEST. (We have included a sample request for information in the link above.)
2. CLick the link below to submit your request. Be sure to fill out the intake information. 
3. Once you are registered, we will send you some additional paperwork via the online portal. 
4. Please allow up to 48-96 hours for a response AFTER WE RECIEVE YOUR PCP NOTE to respond to your request. 

New Patient Requests will be processed as soon as possible.
